Hepatic lipidosis, degeneration with cholestasis in a 6 year old FS DSH cat

A 6-year-old FS DSH presented for persistent lethargy and poor appetite. Physical examination was unremarkable. The blood chemistry profile demonstrated an initially moderately elevated ALT level and then a normal value at the time of the sonogram after S-adenosyl-L-methionine (SAMe) therapy. However, the poor appetite and partial lethargy continued. The PT was slightly prolonged initially but normalized with vitamin K therapy.

Sonographic Differential Diagnosis

Neoplasia and an acute inflammatory hepatopathy. The parenchymal changes are suggestive of a chronic inflammatory pathology.

Image Interpretation

The sonogram reveals diffuse hepatic enlargement with a moderately hypoechoic, slightly coarse parenchyma. The intrahepatic portal vascular walls have a relative increase in echogenicity (short arrow). The serosal margin in the far field is mildly irregular (long arrow).

DX

Moderate, chronic hepatic lipidosis and degeneration, with cholestasis

Outcome

The patient responded well to a lipidosis and hepatic support protocol.

Clinical Differential Diagnosis

Liver pathology: Hepatitis, biliary disease, neoplasia; Pacreatic pathology: pancreatitis, neoplasia.

Sampling

18-gauge US-guided biopsy revealed moderate chronic hepatic lipidosis and degeneration with cholestasis. Liver tissue culture was negative.
