Gastric mass-fibrosing gastritis in a 10 month old MN cat

A 10-month old MN cat was presented for anorexia and weight loss. Blood chemistry revealed a low total protein, hypoalbuminemia, and hyperamylasemia. CBC showed leukocytosis secondary to a neutrophilia, monocytosis, eosinophilia and basophilia, as well as thrombocytosis. Urinalysis showed the presence of hematuria with a urine specific gravity within normal limits. The urine microalbumin was within normal limits.

Sonographic Differential Diagnosis

Gastric mass with epigastric lymphadenopathy and mesenteric root lymphadenopathy. Rule out FIP and lymphoma, mast cell disease or other neoplastic.

Image Interpretation

The gastrointestinal tract presented a 4cm greater curvature annular mass with an associated 2.8cm epigastric lymphadenopathy. The pyloric region appeared free of evident pathology as did most of the small intestine, however minor mesenteric root lymphadenopathy was also noted.

DX

Moderate, chronic-active, eosinophilic, fibrosing gastritis.

Outcome

The patient responded well to azithromycin & prednisolone treatment and was stable 6 months post surgery.

Clinical Differential Diagnosis

GI tract pathology (foreign body, dietary indiscretion, gastroenteritis -viral or bacterial- gastrointestinal parasitism, neoplasia); Pancreatic pathology ( pancreatitis secondary to dietary indiscretion, neoplasia).

Sampling

US-guided FNA, Tru-cutВ® biopsies, and surgical biopsies revealed similar findings: The gastric mass was biopsied and showed moderate, chronic-active, eosinophilic, fibrosing gastritis, while the mesenteric lymph node revealed moderate, chronic active, proliferative, eosinophilic lymphadenitis.Coagulation panel was normal.
