Flattened adrenals in a non-Addisonian 5 year old Golden Retriever dog with vomiting, hematochezia and hypoalbuminemia.

A 5-year-old NM Golden retriever was presented for vomiting. Hematochezia was present on physical examination. The only pertinent abnormality on serum biochemistry was hypoalbuminemia.

Sonographic Differential Diagnosis

Flattened adrenal glands raise concern for potential Addison’s disease. Treatment for gastritis and screening for Addison’s disease would be warranted. No evidence of overt foreign body obstruction was noted.

Image Interpretation

Both adrenal glands were flattened and isoechoic to the surrounding tissue. The right adrenal gland measured 1.85 x 0.45 cm. The left adrenal gland measured 2.6 x 0.41 cm. Given the flattened contour and isoechoic presentation, screening for Addison’s disease would be recommended.

Outcome

The patient is currently doing well without any vomiting or diarrhea.

Clinical Differential Diagnosis

Gastrointestinal symptoms – foreign body, intussusception, IBD, neoplasia, non-specific gastro-enteritis (viral/bacterial/helminths/protozoal/toxins), dietary hypersensitivity. Hypoalbuminemia – chronic hepatopathy, neoplasia, cirrhosis, or Addison’s disease.
