Expansile posterior chamber, lens luxation, and retinal detachment in the right eye of a 14 year old MN Poodle mix

A 14-year-old NM Poodle mix dog with stable renal disease was presented for evaluation of acute onset hyphema in the right eye. Current therapy is Epakitin and Fortekor (benazepril). Abnormalities on physical examination were hyphema, purulent ocular discharge, corneal opacity and edema, and proptosis. CBC and serum biochemistry showed no significant abnormalities.

Sonographic Differential Diagnosis

Given the displacement of the lens, significant trauma or an expansive neoplastic process is most likely.

Image Interpretation

The left eye was imaged with normal anechoic fluid in the anterior and posterior chambers. The lens was in proper position. No evidence of retinal detachment or retrobulbar pathology was noted. However, the right eye presented displacement of the lens with a retrobulbar space filled with echogenic material. This is consistent with blood +/- neoplasia. Areas of retinal detachment were also noted with significant complications. This may be due to trauma or potential neoplasia. Retrobulbar space and optic nerve appeared intact. However, some extension of the pathology of the posterior chamber was noted.

DX

OD: trauma or expansive neoplastic process.

Outcome

None

Comments

Removal of the right eye would be recommended in this case with appropriate biopsies and culture.
