End stage renal dystrophy in a 10 year old MN Maine Coon cat

A 10-year-old neutered male Maine Coon cat with a history of lethargy and vomiting was presented for evaluation of renal failure. Cardiac murmur was present on physical examination. On urinalysis, inappropriate SG (1.014) and hematuria were evident. Blood work showed anemia, azotemia, hyperphosphatemia, and elevated liver enzyme activity. After 24 hours of fluid therapy, there was some improvement in renal values and general patient attitude.

Sonographic Differential Diagnosis

The kidneys presented significant dystrophic changes with interstitial nephrosis pattern and pelvic mineralization. Severe end stage renal dystrophy with infarcts and calculi is likely.

Image Interpretation

The left and right kidneys were severely infarcted with dystrophic mineralization. The right kidney measured 3.16 cm with multiple cortical infarcts. Right kidney pelvic calculus measured 0.56 cm. The left kidney measured 2.0 cm and revealed more severe dystrophic changes.

Outcome

None

Comments

Blood pressure measurements are recommended with urine culture and sensitivity. 72 hour IV fluid protocol and reassessment of the renal values would be recommended. If no clinical response is present, then the prognosis is poor. However, regardless the long term prognosis is guarded to poor.
