Image Interpretation

CT of the abdomen –

The colon is dilated with gas and a mild amount of fecal material. The descending colon is included up to the pelvic inlet. Mild dilation of the extrahepatic biliary ducts is noted. The left lobe of the pancreas presents mild swelling, scalloping contours and non-uniform contrast enhancement.

DX

Dilation of the colon with gas • Possible triaditis with cholangitis and chronic pancreatitis

Outcome

The colonic dilation with gas may be a consequence of maldigestion/indigestion. Possible differential include diffuse inflammatory and less likely neoplastic infiltrates. Note that ultrasound is more sensitive to intestinal wall changes and should be added as a diagnostic measure, especially in case the patient is refractory to symptomatic treatment.
