CT Malignant Nasal Soft Tissue Neoplasia in a 13 year old MN Labrador Retriever

This 13 year old MN Labrador was presented for bloody nasal discharge. This patient has a history of Horner’s Syndrome.

Image Interpretation

CT of the head, plain and post contrast: There is a soft tissue attenuating mass lesion with moderate non uniform contrast enhancement occupying the right nasal cavity. The mass shows infiltrative growth and does not respect the bony borders or borders of the nasal cavity either. It penetrates into the right orbita, right frontal sinus, left nasal cavity and nasal fundus as well as towards the cranial vault. The mass effect causes a moderate right sided exophthalmos. There is severe aggressive bone destruction of the hard palate, maxilla including its alveolar crest, nasal bone, nasal septum, frontal bone, cribriform plate, pterygoid and bony orbita as well as extensive turbinate destruction. The lymph nodes of the head are not included in the scan.

DX

The CT findings are compatible with a malignant nasal soft tissue neoplasia with infiltrative growth and aggressive bone destruction. Likely differential diagnoses include adenocarcinoma, squamous cell carcinoma, lymphosarcoma, melanoma, fibrosarcoma and other.

Outcome

Complete surgical resection unfortunately is not an option here. Next to all other mass effects invasion of the cranial vault has to be expected. Radiation oncology and/or chemotherapy maybe discussed as a palliative treatment options, but would require complete staging with primary tumor biopsy/histology, regional lymph node sampling, chest radiographs and ultrasonographic examination of the abdomen.
