This 7 year old FS DSH cat has a history of chronic nasal discharge and sneeaing. Minimally responsive to antibiotics. Overweight, congested upper airweight. Crea 1.8.

Image Interpretation

CT of the skull: 

Both nasal cavities present a moderate amount of non-gravity dependent, non-contrast enhancing material attached to the nasal turbinates. There is mild destruction of the turbinate and conchal structures of both nasal cavities. The mucosal lining of the nasal cavity is moderately thickened with mildly increased contrast enhancement. In both frontal sinuses a mild amount of non-contrast enhancing soft tissue attenuating material is noted.  In the right tympanic bulla a small drumstick like mineral-attenuating structure emerges from ventromedial aspect of the osseous lining and extends in the lumen, it measures 1.8 x 0.5mm. The left tympanic bulla is unremarkable.  The left and right submandibular and medial retropharyngeal lymph nodes are prominent with mild irregular contrast enhancement, the short-to-long-axis-ratio is maintained normal with < 0.5.

 

Outcome

Viral rhinitis with bacterial superinfection or
primary bacterial rhinitis are the most likely causative agents. However, early stages of
mycotic superinfection (cryptococcal rhinitis) cannot be ruled out entirely. Secondary bilateral reactive lymphadenitis of the tributary lymph nodes is noted.
Recommend rhinoscopy for further evaluation with sampling for culture and
histopathology to check for underlying fungal disease. Long-term antimicrobial
therapy – for at least 4 weeks – should be considered in regards to an antibiotic
sensitivity test. Clinical signs may reoccur in chronic cases.
