This 10 year old MN Shih tzu presented with inability to walk, delayed CP on pelvic limbs, pain present, anal tone present, panniculus present. Responded well to initial medical management then recurred.

DX

• Rightsided extradural space occupying lesion causing compressive myelopathy suggestive for a lateralized disc extrusion of the C3/4 disc • Benign monostotic osteolytic bone lesion within the vertebral bod of C4 compatible with a bone cyst/fat islet Mild dural mineralization

Image Interpretation

CT of the cervical spine- An isoattenuating extradural space occupying lesion is seen obliterating the right and ventral epidural space at the level of the intervertebral disc space C3/4. The spinal cord is displaced and compressed to the left lateral aspect of the vertebral canal. The extradural space occupying effect extends cranially over the entire length of C3 

Outcome

The changes within the epidural space of the vertebral canal are most consistent with rightsided and cranial migration of extruded intervertebral disc material. However, some findings – such as the attenuation pattern – are unusual and therefore the very unlikely potential of differential diagnoses – such as emerging primary or metastatic extradural neoplasia (round-cell neoplasia (lymphoma, plasmocytoma), fibrosarcoma or other) or granuloma – should be considered. If the changes are consistent with the clnical findings and neuroanatomic localization surgical exploration and decompression is recommended.
