CT – Age related changes of the TMJ and 4 jaw quadrants with chronic otitis media in a 11 year old Jack Russell Terrier dog

Case Study

CT – Age related changes of the TMJ and 4 jaw quadrants with chronic otitis media in a 11 year old Jack Russell Terrier dog

The patient is a 11 year old Jack Russell Terrier dog who recently seems painful while eating. History of teeth problems. PE: no abnormalities noted.

The patient is a 11 year old Jack Russell Terrier dog who recently seems painful while eating. History of teeth problems. PE: no abnormalities noted.

DX

Moderate age-related changes in the TMJs and all 4 jaw quadrants with chronic otitis media

Image Interpretation

CT of the head/dental, plain and post contrast – Moderate generalized bone atrophy is noted for all 4 quadrants. Furthermore there are
signs of progressive tooth root resorption, developing ancylosis and mild generalized
paradontal disease. There are no signs of alveolitis, osteitis, tooth root abscessation or
oronasal fistula formation.
Both temporomandibular joints (TMJs) reveal mild signs of age related degenerative
osteoarthropathy. The conformation oft he TMJs is within normal limits.
Right upper quadrant
206, 207 & 210 missing;
Left upper quadrant
206 & 207 missing;
Left lower quadrant
206, 208, 210, 211 missing;
Right lower quadrant
206, 208, 209, 210, 211 missing;
Both the right and left bulla partially contain soft tissue attenuating material which is
dependent on the mildly thickened bulla wall on the left side but extends into the ear canal on the right side. Both ear canals are narrow with moderate generalized wall
thickening and multifocal wall metaplasia.

Outcome

The computed tomographic findings are compatible with moderate age related and
likely incidental changes of the TMJs and all 4 jaw quadrants including generalized
bone atrophy, tooth root resorption with developing ancylosis and mild paradontal
disease. Signs of tooth root abscessation, alveolitis or fistula formation are lacking.
The significance of the findings with regards tot he clinical presentation is
questionable.
There are bilateral signs of moderate chronic otitis media et externa. An emerging
chronic inflammatory polyp is a possible differential diagnosis for the right bulla but is
low for potential. Bilateral otoscopic workup is recommended.
