CT – Abscess formation in the right ventral larynx with cellulitis in a 3 year old Cane Corso

This 3 year old F intact Cane Corso presented for decreased appetite, lethargy and swelling on the ventral cervical region. Abscess drainage with drain placement performed. Persistent swelling. R/O FB penetration

Image Interpretation

CT of the neck/skull – 

The subcutaneous tissue level with C2 and C3 at the right lateral aspect of the neck presents an irregularly contrast-enhancing heterogeneous soft tissue swelling with a central drain and gas content, measuring 7.2 x 3.4 x 5.6 cm. Moderate peripheral fatstranding is noted. A thick walled spherical cavernous lesion is noted at the right ventral aspect of the larynx presenting marked peripheral contrast enhancement sparing the hypoattenuating uniformly soft tissue-attenuating center of the lesion. The lesion measures 5.5 x 3.8 x 4.8 cm. A moderate mass effect on the larynx, that is displaced to the left lateral aspect, is noted. Marked circumferential fat-stranding is noted.

Outcome

The drained abscess at the right lateral aspect of the neck is in remission.
Septic cellulitis in the pertaining soft tissues in both localizations, sialadenitis and
reactive/suppurative lymphadenitis of the regional lymph node.
Surgical drainage of the new-formed abscess is advised. Long-term antimicrobial
therapy based on the results of antibiotic sensitivity testing is essential.
