Chronic cystitis with bladder sand and calculi. Urethral sand and calculi. Cystotomy with bladder wall biopsy is recommended to ensure that only chronic cystitis is present. Sand analysis, as well as both normograde and retrograde flushing would be warranted.
The urinary bladder in this patient presented chronic cystitis pattern with micropolypoid changes at the apex, which were relatively uniform. However, the bladder itself presented 4.0 cm of progressively shadowing sand and debris with urethral sand and small calculi.
The patient will finish antibiotics and continue on S/D diet, but the patient appears to be doing well.
Stranguria – urethral pathology (urolith, urethritis, trauma, stenosis), bladder pathology (urolith, bacterial cystitis, chronic cystitis, polyp).
Organisms on Urine MIC were Escherichia coli and appear sensitive.
