Cholecystolithiasis, Cholangitis, with Post Hepatic Obstruction and Pancreatitis in a 13 year old FS DSH cat

 Presenting Clinical Signs: fever, vomiting and abdominal pain. Weight: 9.8 

Chemistry: ALT 102, bilirubin 1.7

Image Interpretation

The urinary bladder presented a small amount of sand accumulation. The pelvic urethra was imaged 3.0 cm beyond the cystourethral junction and was structurally unremarkable. The common bile duct was mildly dilated at 0.4 cm with a tortuous cystic duct. An inflammatory pattern was noted around the common bile duct. Calculi were noted in the gallbladder and cystic duct. Common bile duct calculi were also noted. The pancreas revealed a dilated duct measuring 0.19 cm. The left limb of the pancreas was hypoechoic to the surrounding fat and mildly irregular. Low grade inflammation is suspected.

Cholecystolithiasis. Cholangitis presentation. Dilated common bile duct. Post hepatic obstruction. Concurrent left limb pancreatitis, low grade. Bladder sand

Comments

Recommend surgical intervention with cholecystectomy and common bile duct lavage +/- bile duct deviation. Urine culture warranted and /or cystotomy at the time of surgery
