Bicavitary and pericardial effusion and lung mass in a 13 year old MN Jack Russell Terrier

A 13-year-old NM Jack Russell terrier dog was presented for evaluation of PU/PD, decreased appetite, and abdominal distension. Blood work showed mild hypoproteinemia and elevated ALT activity.

A 13-year-old NM Jack Russell terrier dog was presented for evaluation of PU/PD, decreased appetite, and abdominal distension. Blood work showed mild hypoproteinemia and elevated ALT activity.

Sonographic Differential Diagnosis

Pericardial and pleural effusion with tamponade and lung mass at the heart base. Significant volume contraction of the heart and tamponade effect was noted. The echogenicity of the pericardial fluid is most consistent with hemopericardium. The liver presentation is consistent with passive congestion and secondary ascites.

Image Interpretation

Transdiaphragmatic view revealed a large amount of pleural effusion. Pericardial effusion was noted and contained echogenic debris. There was variable lung consolidation at the heart base. The abdomen revealed a moderate amount of ascites. The ascites is likely due to passive congestion. Dilated hepatic veins were noted as well as a dilated vena cava. This presentation is strongly suggestive of thoracic neoplasia with obstructive caval disease, although there was no evidence of neoplasia in the abdomen.

DX

Pericardial and pleural effusion with tamponade and lung mass at the heart base.

Clinical Differential Diagnosis

Abdominal distension – fluid, organ enlargement, neoplasia. Pericardial effusion.

Sampling

Pericardiocentesis and pleurocentesis with cytospin is recommended in this patient.
