Annular small intestinal mass in an 11 year old FS DSH cat

An 11-year-old FS DSH cat was presented for the evaluation of clinical signs consistent with upper respiratory disease. An abdominal mass was identified on abdominal palpation. A neutrophilia, lymphopenia, eosinopenia and thrombocytopenia were present on the CBC. Hyperglobulinemia and hyperproteinemia were present on the blood chemistry.

Sonographic Differential Diagnosis

Lymphosarcoma, carcinoma, FIP, complicated inflammatory bowel disease., FIP, mast cell.

Image Interpretation

The gastrointestinal tract was largely normal with prominent mucosa until distal ileum presented an annular type mass deriving from the intestinal wall and involving associated lymphadenopathy.

DX

Annular type mass, distal ileum, involving lymphadenopathy. Intestinal FIP, LSA, or MCT

Outcome

No further information regarding the final diagnosis was available in the patient’s file and the cat was lost to follow up.

Comments

Resection with ileocecal colic anastomosis is possible.

Clinical Differential Diagnosis

Neoplasia (lymphoma, adenocarcinoma, leiomyoma, leiomyosarcoma, mast cell tumor.) Reactive lymph node due to a systemic inflammatory response, pancreatitis causing a mass effect due to severity of inflammation, partial obstruction of the intestinal tract secondary to a neoplasm, foreign body, granuloma, pyogranulomatous inflammation (FIP,) abscess.

Sampling

Fine needle aspirates of the mass were performed, however a definitive diagnosis could not be made. The results were consistent with either feline infectious peritonitis, lymphoma or a mast cell tumor. US-guided biopsy was then performed. Histopathology revealed similar results without definitive diagnosis.
