An 8-year-old SF Border terrier was presented for evaluation of thinning hair coat, PU/PD, and distended abdomen. Laboratory abnormalities included an abnormal dexamethasone suppression test, elevated BUN/Creatinine ratio, and hyperphosphatemia.

Sonographic Differential Diagnosis

Age related adrenal changes. No overt suspicion of neoplasia was noted.

Image Interpretation

The adrenal glands appeared slightly prominent, mildly heterogeneous and slightly nodular. No evidence of capsular expansion or invasion in the phrenic veins were noted. This is considered likely an age related change with the minimal potential of emerging neoplastic event. The adrenal glands measured 2 x 0.7 cm on the left and 2.3 x 0.8 cm on the right.

Outcome

The patient was referred to University of Pennsylvania. The patient had endogenous ACTH performed and was started on Mitotane, Denosyl and Prednisone.

Pituitary-depended hyperadrenocortism, functional adrenal neoplasia
