Adrenal mass and splenic mass in a 12 year old MN Brittany Spaniel

A 12-year-old NM Brittany Spaniel dog was presented for evaluation of lethargy, vomiting, and polypnea. Abnormalities on laboratory testing included anemia, leukocytosis, and elevated BUN.

A 12-year-old NM Brittany Spaniel dog was presented for evaluation of lethargy, vomiting, and polypnea. Abnormalities on laboratory testing included anemia, leukocytosis, and elevated BUN.

Sonographic Differential Diagnosis

Left adrenal gland mass. Concurrent splenic mass. Suspect pheochromocytoma with metastatic splenic lesion or possible separate pathology. There was no evidence of vascular invasion from the left adrenal.

Image Interpretation

The left adrenal gland comprised a mildly complex mass that was deriving from the caudal pole and measured 2.46 cm in width. The left adrenal gland measured 3.96 cm in length and the cranial pole measured 0.82 cm. There was no evidence of vascular invasion. The right adrenal gland was normal and measured 2.5 x 0.6 cm.
The spleen presented a mixed, hyperechoic and hypoechoic mildly complex mass that measured 5.06 x 3.72 cm with similar echogenicity to that of the left adrenal gland. Hyperechoic surrounding fat was noted associated with the spleen. This is suggestive for rupture. Slight, coalesced free fluid was noted.
Rapid view of the heart revealed no evidence of right auricular masses or pericardial effusion.

Outcome

None

Comments

Left adrenalectomy and splenectomy would be recommended if chest radiographs are free of evident pathology. Serial blood pressures are recommended.

Clinical Differential Diagnosis

Splenic disease (neoplasia, torsion, reactive), immune-mediated hemolytic anemia, GI tract blood loss (neoplasia, ulceration, foreign body).
