Acute on chronic pancreatitis in a 12 year old FS Lhasa Apso confirmed with FNA/cytology

A 12-year-old SF Lhasa Apso was presented for evaluation of diarrhea. Abnormalities on CBC were mild anemia, neutrophilia, and monocytosis. cPL was severely elevated ( >2000)

 

Sonographic Differential Diagnosis

Acute on chronic pancreatitis presentation. Minor potential for carcinoma.
Recommend treatment for pancreatitis based on cytology results and type of inflammatory elements assuming that carcinoma is not present.

Image Interpretation

The right limb of the pancreas revealed mixed, hypoechoic, irregular parenchyma with a focal region that measured 2 x 3 cm with extension of hyperechoic surrounding fat enveloping the pyloric outflow and portions of the greater curvature of the stomach. This is most consistent with acute on chronic pancreatitis that extended over to the left base as well. FNAs were performed without complication.

Neutrophilic inflammation (pancreatitis).

Pancreas – acute pancreatitis, chronic pancreatitis, neoplasia, abscessation, cyst
Duodenal obstruction – foreign body, neoplasia, focal peritonitis

Sampling

FNA of the pancreas revealed marked neutrophilic inflammation (pancreatitis).
