A 10-year-old SF Golden retriever was presented for evaluation of acute onset lethargy and vomiting, with a hematocrit of 36%. On survey thoracic radiographs a diffuse multifocal nodular pulmonary pattern was evident.

Sonographic Differential Diagnosis

Prominent hepatic veins and vena cava are due to elevated right sided pressures from pericardial effusion. Pericardial effusion with a huge mass at the junction of the right atrium and the right ventricle is consistent with hemangiosarcoma. The diagnosis is consistent with metastatic hemangiosarcoma effecting the right atrium, spleen and lungs.

Image Interpretation

An echocardiogram was performed and pericardial effusion was noted. The left atrium was normal to small in size. The left ventricle was normal to small in size. The mitral valve was normal thickness and demonstrated normal motion. The right atrium and right ventricle were abnormal in appearance with a large mass invading the wall between the right atrium and right ventricle. The mass measured at least 4.25 x 4.16cm and the mass extended through the wall of the heart into the right ventricle through the valve and up into the right atrium. Contractility was normal as represented by the fractional shortening. The left and right ventricular outflow tracts were normal in appearance with no evidence of stenotic lesions. The tricuspid valve, aortic valve and pulmonic valve were smooth and normal thickness. Aortic flows and pulmonic flows were normal in velocity. There was no evidence of mitral or tricuspid regurgitation. Thoracic radiographs revealed a mild cardiomegaly. There was a diffuse multifocal nodular pattern throughout the lungs.

Outcome

Owners elected euthanasia
