Sonographic Differential Diagnosis

This seems to be a case of L-R shunt. The left ventricle is volume overloaded and has to cope with demand of increased stroke volume. The mitral annulus is consequently dilated leading to slight mitral regurgitation. Flow across the PA is highly turbulent with high pressure backflow – also during diastole. I cannot see a PDA but this is most likely the problem here. Differential diagnoses are: aorticopulmonary window or bronchiopulmonary fistula. An angiography is strongly recommended. The problem should be solved soon to avoid shunt reversal.

Image Interpretation

Severely volume overloaded left ventricle. The mitral valve leaflets show good coadaption and no thickening. But there seems to be still some mild mitral regurgitation. The left atrial size is borderline. The left ventricular outflow tract and aortic valve appear normal, but there is some flow acceleration (2,44 m/s) present which could be due to increased stroke volume (since there is no stenosis visible on 2D). The right ventricle and atrium are a little bit enlarged. The pulmonic valve appears normal on 2D. The craniodorsal part of the main pulmonary artery seems to be enlarged too. Flow across the PA is very turbulent during systole as well as during diastole. There is diastolic backflow noted with a PG of at least 4 m/s. I can see some pulmonary valve insufficiency too.
