A 9-year-old NM Pug mix was presented for evaluation of weight loss and a 5-day history of vomiting and anorexia. A firm swelling was noted adjacent to the os penis.

Sonographic Differential Diagnosis
Intestinal hernia. Potential infection. Recommend surgical intervention for hernia repair. Intestinal resection may be necessary as strangulation is likely. Intestinal biopsies are strongly recommended given the transmural presentation.
Image Interpretation
The gastrointestinal tract revealed variable thickening of the distal small intestine in the caudal abdomen with regional hyperechoic surrounding fat. This is consistent with adhesions and transmural pathology. Reactive fat was noted associated with the intestine and appeared to be herniated in the inguinal area adjacent to the os penis. A trace amount of free fluid was noted in the region.
