Infiltrative pattern in the spleen and liver in this patient. FNAs were performed without complications.
Multicentric lymphadenopathy. Lymphoproliferative disease, lymphoma or FIP like syndrome are the primary differentials.
The spleen was enlarged in this patient measuring 1.5 cm in width and was dramatically hypoechoic with scalloping contour.
The liver was also irregularly enlarged and hypoechoic to the falciform fat, which was minimal owing to cachexia.
Epigastric lymph nodes were slightly enlarged and measured 0.67 cm with pericapsular inflammatory pattern. Aortic lymph node was also enlarged and measured 0.9 x 0.67 cm with a similar pattern. Mesenteric lymph nodes were also enlarged and measured 1-1.5 cm and encompassed the mesenteric artery.
Minor areas of free fluid was noted and coalesced adjacent to the spleen and liver. This is suggestive of an infiltrative pattern and transcapsular fluid accumulation owing to lymphatic obstruction.
