Mediastinal lymphoma diagnosed by FNA of mediastinal lymph nodes in a 4 year old hypercalcemic MN Golden Retriever

A 4-year-old MN Golden Retriever with a history of lethargy and intermittent diarrhea was presented for an office visit. The only abnormality on serum biochemistry was hypercalcemia.

Sonographic Differential Diagnosis

Cranial mediastinal lymph node mass. This is highly suggestive for lymphoma, likely the cause of the underlying hypercalcemia. Thymoma possible given the position of the mass but unlikely given the clinical presentation. Concurrent mild sublumbar lymphadenopathy was also found on abdominal sonogram (not shown).

Image Interpretation

Cranial mediastinal lymph nodes were enlarged and dramatically hypoechoic. This is highly suggestive for lymphoma given the hypercalcemia. This lymph node cluster/mass measured 5.0 cm in width. The largest lymph node measured 4.91 cm. Iliac sublumbar lymph node was also enlarged in this patient and isoechoic to surrounding fat. It measured 2.85 cm.

Mediastinal lymphosarcoma, intermediate cell type

Clinical Differential Diagnosis

Neoplasia (lymphoma/sarcoma/leiomyosarcoma) – lymph nodes, liver, spleen, GI tract, kidney. Granulomatous disease – spleen, lymph nodes, liver, kidney.

Sampling

FNA of mediastinal lymph nodes: microscopic analysis – lymphosarcoma, intermediate cell/prolymphocytic type
