A 9-year-old intact male Pembroke Corgi was presented for evaluation of anorexia and painful swelling of left testicle. On physical examination, the left testicle was swollen; and there was mild scrotal edema and non-painful prostatomegaly. Urinalysis showed pyuria. Abnormalities on CBC and serum biochemistry were mild leukocytosis, neutrophilia, monocytosis, and elevated ALT activity.

 

Sonographic Differential Diagnosis

Epididymitis, prostatitis presentation.
I recommend culture of the epididymis in this patient as well as urine culture. If Brucellosis is found, then spinal radiographs would be recommended to assess for any vertebral lesions. Aggressive antibiotic protocol would be recommended based on culture results.

Image Interpretation

The prostate was mildly enlarged with heterogenous parenchymal changes. The prostate measured 4.15 cm in width. This is consistent with BPH and prostatitis. Parenchymal striations were noted within the prostate. This is suggestive for inflammation and edema. The testicles presented slightly heterogenous parenchyma. The left testicle revealed a thickened epididymis with mixed, echogenic changes. Cystic structures were noted within the epididymis of the left testicle and is suggestive of micro abscessation.
