A 10-year-old NM Chihuahua that had been recently treated for pancreatitis and DKA was evaluated for ongoing anorexia. Serum biochemistry showed hyperglycemia and severely elevated ALP and GGT activity.

Sonographic Differential Diagnosis

Pancreatic abscess. Extensive pancreatitis, duodenitis, and potential for post hepatic obstruction.
Ultrasound-guided drainage of the abscess was performed without complication. Enrofloxacin was injected directly into the abscess without complication. Recommend recheck sonogram in 4 days prior to discharge. 24 hour n.p.o. protocol, followed by trickle feeding is recommended. Gastrointestinal protectants, aggressive antibiotics such as injectable Enrofloxacin and Ampicillin combination would be recommended as well as Ursodiol therapy. Attention should be paid towards any rise in bilirubin values. Plasma expanders and plasma transfusion would be ideal in this case.

Image Interpretation

The pancreas in this patient presented mixed, echogenic abscess that measured 5×2 cm with hyperechoic surrounding fat. This appeared to be located in the base of the right limb. Color flow Doppler shows uneven distribution of vascularity. No vessels are seen in the abscessed portion of the pancreas.
