An 18-year-old NM DSH was presented for evaluation of anorexia, lethargic, and ascites. Abnormalities on CBC and serum biochemistry were leukocytosis, marginal hypoalbuminemia (2.4), elevated BUN, and hyperglycemia.

An 18-year-old NM DSH was presented for evaluation of anorexia, lethargic, and ascites. Abnormalities on CBC and serum biochemistry were leukocytosis, marginal hypoalbuminemia (2.4), elevated BUN, and hyperglycemia.

 

Sonographic Differential Diagnosis

Carcinomatosis type presentation. Lymphomatosis and FIP are both possible. Concurrent liver masses, metastatic pattern. Suspect pancreatic carcinomatosis with metastatic spread. Ultrasound-guided FNA of the liver and pancreas were performed.

Image Interpretation

The pancreas was hypoechoic and irregular with nodular omental changes. The pancreas was surrounded by free fluid. This is strongly suggestive for pancreatic carcinomatosis. The liver presented multiple, mixed echogenic masses with free fluid.
