A 13-year-old SF Labrador Mix was presented for evaluation of recent onset stranguria. Physical examination and CBC were both normal. Urine analysis showed mild hematuria and +1 protein. Abnormalities on serum biochemistry were elevated ALP (432) and GGT (16) activity.

Sonographic Differential Diagnosis

Left pancreatic mass appears resectable. Granuloma, carcinoma, other neoplasias are all possible.

Image Interpretation

The left pancreatic limb revealed a 7 x 4.76 cm hypoechoic to echogenic and mildly complex mass. The mass was likely deriving from the pancreas, possible adjacent lymph node, but more likely pancreas. Focal mineralization was noted.

Stranguria/hematuria – uroliths, bacterial cystitis, neoplasia, granulomatous cystitis, polyploid cystitis
Elevated ALP/GGT – cholecystitis, mucocele, partial gall bladder obstruction (neoplasia, duodenal/pancreatic disease), chronic hepatopathy, hepatic neoplasia

Sampling

FNAs were performed without complication.
