04_00530 Annabelle U Pyloric thickening, adrenal mass

A 10-year-old SF Bassett with a heart disease was presented for evaluation of decreased appetite and weight loss. The only abnormality on physical examination was a grade IV heart murmur. Current therapy is enalapril, furosemide, and pimobendan. On serum biochemistry, low-normal BUN (7.2) and mildly elevated ALT (153) was evident. Small liver and a thickened stomach were evident on abdominal radiographs, whereas thoracic radiographs were similar to those taken previously.

 

Sonographic Differential Diagnosis

Hypertrophic pyloric gastropathy with minor regional inflammation. Ideally endoscopy would be performed for mucosal biopsies. If sampling is not an option then a clinical trial for Helicobacter, gastritis with slurry feeding of a hydrolyzed diet would be recommended as well as gastrointestinal protectants and coverage for Helicobacter.

Image Interpretation

The pyloric outflow was thickened with no loss of detail. However, hypertrophied mucosa was noted with hypertrophied muscularis with most aspects of hypertrophic pyloric gastropathy. There was no overt evidence of neoplasia noted. Fluid accumulation was noted in the gastric fundus owing to partial obstructive pattern.
