A 9-year-old SF DSH was presented for evaluation of anorexia. Urinalysis showed SG of 1.048 and pyuria.

A 9-year-old SF DSH was presented for evaluation of anorexia. Urinalysis showed SG of 1.048 and pyuria.

Sonographic Differential Diagnosis

Proximal colonic wall thickening with regional lymphadenopathy.
Early infiltrative splenic pattern. This is suggestive for multicentric lymphoma or similar round cell neoplasia. Guarded prognosis.
Granulomatous intestinal disease or FIP is also possible with reactive spleen.

Image Interpretation

The gastrointestinal tract revealed an intestinal mass at the ileocecal valve. This appeared to be proximal colon. FNAs were performed without complication. Regional lymphadenopathy was also noted.
The spleen was enlarged and micronodular measuring 1.15 cm.

Clinical Differential Diagnosis

UTI – bacterial cystitis, pyelonephritis
Anorexia – GIT (neoplasia, foreign body, IBD), pancreas (pancreatitis, neoplasia), neoplasia, abscessation, granulomatous disease

Sampling

FNA of the GI revealed pyogranulomatous inflammation.
