A 9-year-old SF DSH was presented for evaluation of abdominal pain, anorexia, and lethargy. Bacterial cystitis was present on urinalysis.

A 9-year-old SF DSH was presented for evaluation of abdominal pain, anorexia, and lethargy. Bacterial cystitis was present on urinalysis.

Sonographic Differential Diagnosis

Duodenal obstruction with hard foreign body.

Image Interpretation

The upper gastrointestinal tract revealed a dilated stomach and dilated duodenum with a 1.3 cm shadowing structure. This is consistent with a nut such as pistachio or similar. Immediate exploratory surgery is recommended with enterotomy. Minor areas of intestinal wall thickening and hypertrophied muscularis were noted.
