A 9-year-old NM DSH was presented for evaluation of chronic vomiting and inappropriate urination, with more recent hematemesis. Additional history was therapy with Metacam and no weight loss.

A 9-year-old NM DSH was presented for evaluation of chronic vomiting and inappropriate urination, with more recent hematemesis. Additional history was therapy with Metacam and no weight loss.

Sonographic Differential Diagnosis

Early gastric infiltrative pattern. FNA were performed without complication. Early lymphoma is suspected.

Image Interpretation

The gastric wall revealed a hypoechoic infiltrative pattern with pyloric outflow that measured 1.73 x 0.81 cm with loss of detail. This meets neoplastic criteria and necessitates ultrasound-guided FNA. Lymphoma is the primary differential in this case. Mast cell disease, carcinoma, or complicated inflammatory disease are all possible.

Sampling

Cytology of the stomach revealed lymphosarcoma.
