A 10-year-old NM German Wirehaired Pointer was presented for evaluation of acute onset vomiting and lethargy. Blood work showed elevated amylase, lipase, cPL, and liver enzyme activity. Survey radiographs were suggestive of a linear foreign body.

Sonographic Differential Diagnosis

Linear foreign body pylorus to small intestine. Upper gastrointestinal obstruction. Immediate exploratory surgery is recommended with foreign body removal and gastrointestinal biopsies to rule out concurrent chronic disease. Prognosis is good with immediate exploratory.

Image Interpretation

The gastrointestinal tract revealed a linear foreign body and intestinal plication. This was noted medial to the spleen. Other portions of small intestine were empty. Therefore, this is not likely involving the ileocecal valve. There was no evidence of peritonitis noted. There was some dilation of the stomach noted with anchoring of the linear foreign body in the pylorus. The colon appeared unremarkable. A minor amount of reactive fat was noted associated with the small intestine.

Outcome

Exploratory was performed and revealed plastic chew toy, sock, threads and grass that were removed from the stomach.

Clinical Differential Diagnosis

GIT – foreign body, acute gastro-enteritis (viral, bacterial, toxins, dietary indiscretion) neoplasia, IBD
Pancreas – pancreatitis, neoplasia
Liver – acute hepatitis (viral, bacterial, toxins, trauma)

Sampling

Histology of the stomach revealed moderate pleocellular and erosive gastritis with mild fibrosis. Histology of the liver revealed mild lymphoplasmacytic cholangiohepatitis.
