An 8-year-old NM Labrador was presented for evaluation of chronic vomiting Urinalysis was normal. Abnormalities on CBC and serum biochemistry were neutrophilia, monocytosis, hypoalbuminemia (1.4), and elevated cPL (560).

Sonographic Differential Diagnosis

Distal duodenal thickening.
Variable small intestinal wall thickening. There is a strong potential for emerging lymphoma versus inflammatory bowel and spontaneous necrosis.

Image Interpretation

Distal duodenal thickening of 1.0 cm was noted along with pericapsular inflammatory pattern. The distal duodenum appeared to lose detail. This is most consistent with inflammatory bowel disease and protein losing enteropathy with focal areas of emerging neoplasia such as lymphoma. Spontaneous necrosis is also possible. FNAs were performed upon the intestinal tract without complication.
