03_00548 Sandy H Mucocele

An 11-year-old SF Canine mix with a history of eating bones was presented for evaluation of a 4 day history of progressive lethargy, anorexia, vomiting, and small volume diarrhea.  On physical examination, she was febrile, dehydrated, and the abdomen was tense. CBC was within reference range. Abnormalities on serum biochemistry were elevated ALP (2000) and ALT (856) activity and total bilirubin (3.9), and abnormal cPL snap test.

 

Sonographic Differential Diagnosis

Severe inflamed gallbladder mucocele.
Mucoduct with regional inflammation extending to the right pancreatic limb.

Image Interpretation

The gallbladder in this patient presented a severe mucocele formation with a pericystic inflammation and slight coalescing free fluid at the apex. The common bile duct was also dilated at 0.8 cm. This is a surgical emergency with inflamed gallbladder mucocele and mucoduct.

Liver – acute hepatitis (viral, bacterial, toxins), neoplasia, abscessation, trauma
Gall bladder – cholecystitis, mucocele, neoplasia, obstruction (lith, duodenal/pancreatitic disease)
Pancreas – pancreatitis, neoplasia
GIT – foreign body with perforation, ulceration
