03_00521 Chili M Liver mass

An 8-year-old SF Chihuahua was presented for evaluation of mild lethargy, PuPd and possible cystitis that had not responded to antibiotic therapy. Urinalysis showed SG 1.007 and proteinuria. Abnormalities on CBC and serum biochemistry were neutrophilia, elevated ALT (551) and ALP (532) activity, and elevated urine cortisol/creatinine ratio.

 

Sonographic Differential Diagnosis

Large, left sided liver mass extending to the gallbladder. This is potentially resectable. CT evaluation is recommended. Aggressive full left lobectomy would be necessary in this patient. However, it appears to have margins that are clearly separate from the diaphragm even though very close to the diaphragm. The right liver was clean as was the portal hilus. Hepatocellular carcinoma is suspected.

Image Interpretation

The liver presented a moderately complex echogenic and cystic mass that occupied most of the left liver adjacent to the diaphragm with likely adhesions. The mass measured 9.7 x 6.4 cm. The hepatic mass appeared moderately vascular. The gallbladder was deviated to the right. The right liver appeared largely unremarkable.

Adrenal – pituitary-dependent Cushing’s disease, neoplasia (carcinoma, functional adenoma)
