A 6-year-old SF Cockerlier was presented for evaluation of acute vomiting and sparse haircoat. On physical examination icterus, dental disease, and tacky mucus membranes were present. Abnormalities on CBC and serum biochemistry were mild anemia, leukocytosis, lymphopenia, elevated globulins, bilirubin, and liver enzyme activity (AST 382, ALT 1546, SAP 3135).

Sonographic Differential Diagnosis

Inflamed gallbladder mucocele with cholangiohepatitis.

Leptospirosis titers should be obtained. However, immediate surgical intervention with cholecystectomy, common bile duct lavage and liver biopsy would be recommended.

Image Interpretation

The liver revealed increased portal markings. The gallbladder in this patient was inflamed with striating bile and hyperechoic surrounding fat. This is consistent with inflamed mucocele.

DX

Inflamed gallbladder mucocele with cholangiohepatitis.

None

Liver – acute hepatitis (viral, bacterial, toxins), neoplasia, chronic-active hepatitis, abscess, granulomatous disease
Gall bladder – cholecystitis, mucocele, obstruction (lith, neoplasia, duodenal/pancreatic disease)
Pancreas – pancreatitis, neoplasia
