A 16-year-old NM Poodle was presented for evaluation of vomiting. Abnormalities on serum biochemistry were severely elevated liver enzyme activity and cholesterol, elevated bilirubin, urea, and hypoalbuminemia.

A 16-year-old NM Poodle was presented for evaluation of vomiting. Abnormalities on serum biochemistry were severely elevated liver enzyme activity and cholesterol, elevated bilirubin, urea, and hypoalbuminemia.

Sonographic Differential Diagnosis

Gallbladder mucocele with minor inflammation. This is a surgical emergency. There was no evidence of bile peritonitis noted at this time. However, given the level of distension of the gallbladder and cystic duct, I recommend immediate cholecystectomy with common bile duct lavage.

Image Interpretation

The gallbladder in this patient was significantly dilated with suspended, immobile debris. There was a mild, hyperechoic, inflammatory pattern around the gallbladder. The gallbladder presented a double layered wall as well. The cystic duct and proximal common bile duct were dilated and filled with inspissated sludge. The common bile duct measured 0.33 cm, which is at the high end of normal.
