Sonographic Differential Diagnosis

Likely hepatic lipidosis, probable low grade underlying concurrent inflammatory hepatopathy. Mild potential for lymphoma. 25-gauge FNAs were performed and submitted for cytology. Recommend feeding tube placement and treatment for hepatic lipidosis until FNAs are assessed. Guarded long term prognosis.

Image Interpretation

The hepatic parenchyma was uniformly hyperechoic without disruption of architecture. The liver was enlarged. No masses were noted. the gall bladder and common bile duct were normal. This presentation is most consistent with hepatic lipidosis of variable degree with the minor potentially for underlying lymphoma or inflammatory hepatopathy that is not structurally evident at this time. The potential for these latter pathologies would be based on hepatic enzyme elevations and clinical profile. FNAs were performed.

DX

Marked lipid vacuolar hepatopathy.

Outcome

A feeding tube was placed and the patient seemed to be doing better.

FNA revealed marked lipid vacuolar hepatopathy.
