– 21yr old MN DSH with history of CRF had two episodes or crying, collapse and open mouth breathing on the weekend

– recovered well from both and ate normally afterwards

– no heart murmur is present

– VERY fractious therefore only able to get a couple of views of the heart but was able to determine that the LA was normal in size so ATE due to heart disease unlikely and CHF unlikely

– moderate pericardial and pleural effusion present

– I am suspicious of a possible mass in the IVS. Could this be lymphoma (or other neoplasia)?

Jaquie the images are too dark to say other than there is pc effusion and the left heart isnt the problem. Can you lighten them up on your end or send me th edicom folder and I can adjust it in osirix

I don’t think I can lighten them on my end but I can send a dicom. I scanned through  the fur on this one as he would have been impossible to clip without him completely freaking out, whichly likely didn’t help! I’ll work on getting you a DICOM.

JP I lightened up and slowed down the video and reloaded it for you and yes a focal hypoechoic bulge is coming out of the IVS and witht he pc effusion I am thinking cardiac lymphoma. You should just fna the septum:) Kidding actually if you can sample the pc effusion you may get th elsa dx. Also check the abdomen for anything else to sample that may say lymphoma. Way coiol case we can surely use this as case of the month if you get a definitive dx.

 

FYI to make adjustments after the fact its good to have osirix just import the dicom and adjust the image and export/save as .mov. Osirix is freeware unless you get the enhanced 64 bit version that I use which is pricey at $400/year but for occasional use the free 32 buit version is fine. You can view your rads CT MRI on this as well its the best dicom viewer out there in my opinion.
