Carcinomatosis?

– German Shorthair Pointer of unknown age (adult) MN presented for severe ascites

– had been clinically well until presentation

– bloodwork ALB 19, mild neutrophilia, lymphopenia

– 4 litres yellow-tinged foamy fluid removed via abdominocenteis – cytology unremarkable

– complex mass in the right liver, but normal liver tissue seen on left side;omentum hyperechoic and nodular

– SI normal – no mucosal striations; lungs have lung-rockets or comet-tail artifact with no pleural effusion

I was expecting to find evidence for a PLE but this looks like carcinomatosis to me – can this also explain the low albumin?

Looks like carcinomatosis to me. Albumin of 1.9 would probably not be low enought to cause this ascites. I suspect it is a modified transudate. Many times when I send the fluid in for analysis it comes out non diagnostic. Tumors just don’t seem to exfoliate. 

Hypoalbuminemia:

1. Liver disease

2. Protein losing enteropathy

3. Protein losing nephropathy

4. Fluid shifts in the body (probably the cause in this dog)/severe trauma

No good outcome in my opinion for this presentation. 

We can wait and see what others have to say

Diffuse liver disease and

Diffuse liver disease and Ln/panc masses so the ascites is from 1)portal hypertension or 2) lymphatic obstruction or both and a low oncotic pressure but you have to have albumin of < 1.5 for fluid to form on its own. Fna anything in there should give the dx. Badd joo joos in there:(

I am not doubting your interpretation EL- but how do you know the masses are pancreatic?
