Acute Renal Failure

Lily is a 3 year 10 month old DSH that come in to our clinic with a BUN of 127, Creat 12 and Phosphorus over 16.

No history of poision ingestion. She had a denal at our clinic with horrible teeth. Multiple extractions. One shot of Mloxicam was given (proper dosing) and Buprenorphine was dispensed. This was over 3 months ago.

No history of poision ingestion. She had a denal at our clinic with horrible teeth. Multiple extractions. One shot of Mloxicam was given (proper dosing) and Buprenorphine was dispensed. This was over 3 months ago.

A referral was made to the local Blue Pearl shere she was diuresed for a few days. All of her numbers have come down nicely but she remains in early stage 2 CKD. Urine culture was negative. US at the local BP only indicated the possiblity of pyelonephritis/pylectasia. Never an increase in WBC- but there was some dilation of the renal pelvis (? fluid diuresis).

Lily has been on a renal diet and is just finishing Clavamox.

I recently saw her and looked at her kidneys. I am only supplying 2 long axis cines and stills.

What I see and want your opinion:

1. L kidney is normal in size. Irregular capsule maybe consistent with infrarcts. Not sure what the density is on the caudal pole

2. R kidney is smaller with some loss of contrast between the medulla and cortex

No stones were seen and the bladder was clean.

first video small infarct at 2 o clock and second video caudal cortical collapse. Some sort of acute insult and this is the sequelae… toxin despite negative history of exposure, infectious, some sort of vascular event. Otherwise kidneys look pretty good.
