Abnormal Left Kidney & Spleen

Mardi is a 10 pound 9 1/2 year old male neutered Chihuahua with a recent history of weight loss, lethargy and inappetence.

Mardi is a 10 pound 9 1/2 year old male neutered Chihuahua with a recent history of weight loss, lethargy and inappetence.

He was seen at another clinic. One of my technicians brought him in for a second opinion (her cousins dog). Abdominal x-rays revealed an grossly enlarged mass effect in the region of the L kidney. I am submitting images of the L kidney and spleen. The spleen was enlarged and coarse in texture and hypo-echoic. Splenic vasculature seemed to be OK. The L kidney was grossly enlarged with a loss of normal architexture and a grossly dilated renal pelvis. I was wondering if somone could give me an ultrasonographic description and a list of differentials.

The spleen is inlarged

 
 

The spleen is inlarged with scalloping contour, hypoechoic parenchyma and pericapsular echogenic fat suggestive for infiltrative neoplasia. Splenitis-fungal or bacterial less likely.

The kidney is swollen hypoechoic, irregular and pericapsular inflammatory fat with disrupted pelvis and pyelectasia and dilated proximal ureter. Strongly suggestive of neoplasia, less probable obstructive ureteral disease.

FNA 25 g the right renal cortex and spleen likely round cell neoplasia-LSA or similar.

Thank you EL. This was how I saw this. I was not allowed to do a FNA. I suspect this dog will go to her regular vet for euthanasia.

Yes that pericapsular inflammed fat pattern is saying I need a needle! Whatever that echogenic fat is attached to is the problem. Remember on these that fungal looks like neoplasia. Eeven had a histoplasma case in NJ once that looked like this and we don’t have histoplasma… the dog had traveled.
