Severe renal dysplasia in a 5 month old, FS, Dogo Argentino

A 5-month-old SF Dogo Argentino was presented for evaluation of PU/PD, weight loss, and recent onset vomiting. Abnormalities on CBC and serum biochemistry were mild anemia, severe azotemia (BUN 256, creatinine 9.4) and severely elevated inorganic phosphate (20.8). Urine culture was negative.

Sonographic Differential Diagnosis

Differential Diagnosis: Severe renal dysplasia with minimal blood flow.

Normal GI, yet the stomach was dilated with fluid with edema consistent with concurrent
uremic gastritis.

Image Interpretation

Kidneys in this patient were significantly subnormal in size with disrupted architecture as well as thickened irregular cortices with complete lack of corticomedullary junction and displaced dilated renal pelvis in both kidneys. Left kidney measured 4.11 cm. Right kidney measured 3 cm with similar changes the left. This is most consistent primary renal dysplasia. Power Doppler assessment revealed minimal blood flow.

Examination of the gastrointestinal tract revealed intestine free of stasis, of normal wall thickness, acceptable curvilinear mural detail, and peristaltic activity. The stomach was dilated with fluid with edema consistent with concurrent uremic gastritis. No obstructive or overt infiltrative disease was noted. No abnormal lymphatic activity was noted and the abdomen was free of masses and pathological fluid.

DX

Severe renal dysplasia with minimal blood flow

Outcome

Renal biopsy would be necessary to confirm the diagnosis, yet these are strongly suggestive for primary renal dysplasia/congenital defect.
