CT – Chronic intervertebral disc extrusion with compressive myelopathy in a 9 year old FS Cavalier King Charles Spaniel

This 9 year old FS Cavalier King Charles Spaniel has a 3 month history of sporadic hind limb paresis, which has worsened in the past 2 weeks. Slightly overwieght, CP deficits hind limbs.

Image Interpretation

CT of the spine – 

 Cranial to the caudal vertebral endplate of T2 a round geographic osteolytic area is noted, consistent with an island of fatty bone marrow . 

 

 Level with L1/L2 the intervertebral disc and the dorsal longitudinal ligament present focal mineralization as well as mild mid sagittal protrusion into the ventral epidural space of the vertebral canal. The dural tube is focally compressed at this level. The intervertebral disc space L1/L2 is mildly collapsed. 

A mild spondylosis formation is noted at the ventral aspect of the neighboring vertebral endplates of L1/L2 and L2/L3. 

DX

chronic intervertebral disc extrusion L1/L2 with compressive myelopathy.

Outcome

The compressive myelopathy may be responsible for proprioceptive deficits. However, paresthesia is not a typical sequela of compressive myelopathies with the pertaining neuroanatomic localization. Moreover, the incidence of Chiari-like malformation with
Moderate chronic intervertebral disc protrusion L1/L2 with mild to moderate compressive myelopathy
 Osteoarthrosis both coxofemoral joints due to hip dysplasia, L>>R
 Primary secretory otitis media bilaterally PSOM – a common incidental finding in many brachycephalic breeds, especially common in CKCS
 Spondylosis deformans
 Degenerative intervertebral disc disease C4/C5
Syringomyelia and secondary paresthesia syndromes is high in CKCS of that age.
An MRI study of the head and cervical spine (may want to include lumbar spine as well to rule out lower motor neuron myelopathy of the hind limbs as well) is strongly recommended for further definition if the clinical signs persist.
The current CT study does not reveal evidence of degenerative lumbo-sacral stenosis.
