DX

Gastritis pattern with regional lymphadenopathy

Image Interpretation

Hyperechoic, penetrating echoes were noted in the pyloric outflow. This is suggestive for ulcer. Regional inflammation was noted throughout the pyloric outflow. The echogenic penetration within the wall remained in the mucosal layer. No submucosal changes were noted. A focal 2.0 cm region of the lumen in the pylorus revealed an isoechoic structure. This may be a mucosal or a tubular foreign structure may also be present. Epigastric lymph nodes were slightly enlarged, rounded and measured 1.08 cm.

Outcome

This is likely reactive. Possible luminal foreign body. 2.0 cm structure appeared to be present or irregular mucosal hypertrophy. Mucosal ulcers are suspected. Endoscopy is recommended in this patient for further investigation. Gastrointestinal protocol would be warranted. If sampling is not an option a recheck sonogram would be recommended in 48 hours.
