04_00540 Satchmo R Linear FB

A 9-year-old NM Boxer with a history of previous foreign bodies and dietary indiscretion was presented for evaluation of acute onset listlessness and dehydration.  Spec CPL was positive. Survey radiographs revealed obstructive small intestinal pattern and a volume-contracted heart.

 

Sonographic Differential Diagnosis

Intestinal linear foreign body with accordion pleating and emerging peritonitis.
Immediate exploratory surgery is recommended.
The reactive fat and early peritonitis is minor at this point; however, when I see this sonographically I consider it a surgical emergency. No overt free fluid was noted at this time.

Image Interpretation

The small intestine in this patient presented a shadowing fabric type linear foreign body with periserosal inflammatory pattern. Reactive fat was noted around the hyperperistaltic dilated small bowel. Artifact was noted in the stomach as well. Accordion pleating of bowel was noted with linear foreign body obstruction.
