A 10-year-old SF DSH was presented for evaluation of anorexia, lethargy and pyrexia. Abnormalities on CBC and serum biochemistry were anemia (24%), low BUN, and hypoproteinemia with hypoalbuminemia (2).

 

Sonographic Differential Diagnosis

Infiltrative intestinal mass. Intestinal lymphosarcoma pattern with secondary protein losing enteropathy.
Regional lymphadenopathy. There is a strong potential for early splenic and hepatic involvement. Ultrasound-guided FNA of the intestine and liver would be recommended. Very guarded prognosis.

Image Interpretation

An intestinal mass was noted in this patient and extended for approximately 4.0 cm with complete disruption of architecture. This is strongly suggestive for underlying lymphoma. Reactive mesentery was noted throughout the mid abdomen in this patient.
Caudal abdominal lymph node was enlarged, irregular and hypoechoic with distorted architecture. The lymph node measured 3.3 x 1.9 cm.
