Case Study

07_00177 The Dude S Adrenal mass

A 7 year old, neutered male feline was presented for anorexia, lethargy, decreased frequency of defecation with no dyschesia. Hospitalized on fluids with no improvement. Altered CBC values: Lymphocyte 510. FeLV/FIV both negative. UA specific gravity 1.056 otherwise NSF. Spec fPL 0.7 Chem was wnl.

Sonographic Differential Diagnosis

The ultrasonographic findings are compatible with a right adrenal gland mass – such as
adenoma or adenocarcinoma – and localized retroperitoneal effusion. The effusion may
be hydrostatic, paraneoplastic or hemorrhagic. Vascular invasion was not evident. Primary hyperaldosteronism/Conn’s syndrome is one of the most common
adrenocortical disorders in cats.

Image Interpretation

Left cranial and caudal pole measured below 0.45 cm in height. The right adrenal
gland presented severe asymmetrical enlargement up to approximately 3 x 2 cm and
parenchymal heterogeneity with loss of the regular echoarchitecture. The outline of the right adrenal gland was irregular. The surrounding retroperitoneal fat presented
increased echogenicity and there was localized retroperitoneal effusion.
