RADS – Disturbed Intestinal Motility Pattern with Functional Ileus in a 3 year old MN DSH cat

Presenting Clinical Signs:  lethargy, inappetance, fleas. 

Physical Exam: EENT – squinting, yellowish nasal discharge; saline in nose does not make him sneeze PLN – normal HL – clear –  Abdomen – fluid, diarrhea greenish mucoid despite all meds (metronidazole, propectalin); possible long item in bowel palpated dorsally.   URTI, Diarrhea,  Anemia, Weight loss, Anorexia

Image Interpretation

right lateral and VD thorax and abdomen – The patient was thin.
Osseous structures: There was a pectus excavatum malformation of the caudal sternum shifting the cardiac silhouette to the left and displacing the diaphragmatic cupola caudad.
Intrathoracic and -abdominal structures: The cardiac silhouette was normal for size and shape. There was no mediastinal widening. The major and pulmonary vessels were within normal limits. The lung was underperfused.
The serosal detail was mildly reduced. The liver protruded mildly beyond the costal arch, but the lobar margins were pointed. The gallbladder was moderately to severely distended.
The spleen and kidneys were normal for size, shape and opacity.
The small intestine (red arrows) contained gas and fluid. There were pronounced segmental contractions and luminal accumulations of small gas bubbles. The small intestinal loops presented mild generalized widening and rigid tone. There was no segmental small intestinal dilation. The large intestine (yellow arrows) was empty for fecal material and mainly contained gas. There were pronounced segmental contractions and overall spasticity. The luminal outline of the descending was lost immediately cranial to the pelvic inlet. There was no dilation noted cranial to this site.

DX

The radiographic findings are compatible with a disturbed intestinal motility pattern with possible mild functional ileus

Comments

  There is no sign of complete mechanical intestinal obstruction at this point. Primary intestinal disorders such as infection, chronic inflammatory or neoplastic infiltrate have to be considered as well as toxic/metabolic or neurogenic myenteric disease. Further evaluation of the intestinal morphology and monitoring of real time motility pattern by ultrasonography is recommended. 

The radiographs of the thorax comfirmed a hypovolemic state.
