The patient presented with an acute onset para paresis.

Image Interpretation

CT of the cervical spine: There was a moderate intervertebral disc herniation level with C6/7 causing compressive myelopathy. The vertebral canal diameter was occupied by 40%. The herniated disc material was centered over the intervertebral disc space and located within the ventral epidural space. There was significant dorsal deviation and thinning of the dural tube. The intervertebral disc space showed wedge shaped narrowing. Mild end plate sclerosis and mild new bone formation at the articular processes are compatible with degenerative arthropathy and indicate chronicity. There was a step formation of the vertebral canal floor level with C5/6. There was mild ventral flattening of the cranial vertebral endplates of C6 and C7. The intervertebral disc C5/6 presented moderate herniation into the ventral epidural space centered over the intervertebral disc space. Mild to moderate compressive myelopathy was present. The vertebral canal diameter was occupied by 30%. The intervertebral disc space showed wedge shaped narrowing. Minor disc protrusions were noted along all intervertebral disc spaces of the cervical spine and cervicothoracic transition. No significant compressive myelopathy was noted at this point.

DX

High resolution CT of the head revealed disc associated caudal cervical spondylomyelopathy. The main findings were chronic protrusions of the intervertebral discs C5/6 and C6/7 causing moderate compressive myelopathy.

Outcome

A dynamic component of spinal cord compression has to be considered and should be clarified by flexion extension myelogram and myelo CT before surgery is planned. The prognosis is guarded as signs of chronicity are present which frequently is accompanied by spinal cord gliosis preventing restoration of the nerve tissue. Moreover, the fact that two adjacent intervertebral disc spaces are affected contributes to the complexity of the disease as fulcrum effects need to be considered when surgical decompression and stabilization techniques are used.
